Fees for Services

 

Intake Session 

Individual: $160 per session (70-80 minutes)  You may pay with your Credit Card through PayPal by clicking this button:

Intake forms must be received at the beginning of the first session for any service - or may be submitted prior to the first session. They are available through the Forms link on the left. Please note that I do not accept credit cards in the office, only cash or check unless you use PayPal to pay 24 hours in advance of the session.

 

Individual Therapy *

$125 per session (45-50 minutes) Longer sessions are pro-rated for the additional time. You may pay with your Credit Card through PayPal by clicking this button:

Couples Therapy *

Telephone Counseling *

$75 per session (45-50 minutes) – I will call you at the specified appointment time. The fee must be paid in advance. You may pay with your Credit Card through PayPal by clicking this button:

Internet Counseling *

$75 for three email exchanges. The fee must be paid in advance. You may pay with your Credit Card through PayPal by clicking this button:

Web-Cam Counseling *

$75 per session (45-50 minutes) We will connect through Skype. The fee must be paid in advance. You may pay with your Credit Card through PayPal by clicking this button:

Group Therapy

$35 per session (4 weeks @ 90 minutes) You may pay with your Credit Card through PayPal by clicking this button:

Assessment

$150 per hour (including clinical interviews, collateral interviews, document review, psychological testing, report writing, and transportation (if required) Assessment components are determined on a case by case basis, they may, or may not, include additional fees for polygraph or phallometric examinations as part of the assessment. Polygraph Examinations

$350 for single issue exams. You may pay with your Credit Card through PayPal by clicking this button:

$500 for two issue exams. You may pay with your Credit Card through PayPal by clicking this button:

$500 for full sexual history or fidelity testing, You may pay with your Credit Card through PayPal by clicking this button:

$500 for sex offender exams (maintenance or monitoring). You may pay with your Credit Card through PayPal by clicking this button:

$350 for subsequent exams as needed. You may pay with your Credit Card through PayPal by clicking this button:

 

Phallometric Testing

$500 per exam session. You may pay with your Credit Card through PayPal by clicking this button:

 

* Note: Individual or Couple sessions extending beyond 50 minutes or any telephone consultations over 10 minutes will be charged on a pro-rated basis for the additional time.

 

The charge for a returned check is $25.00. The fee is generally paid at the end of each session. You understand that you are responsible for the above fees, regardless of whether your insurance covers the services provided or whether the insurance check is sent to you directly.

 

Please bring up and discuss any fee concerns with me as they occur.

 

In the event of failure to pay for professional services, you agree to pay for the costs of collection and reasonable attorney’s fees and expenses including those of appeal. For any future legal or court-related services, you agree to pay the prevailing hourly fees.

 

To avoid being charged for a missed session, you agree to notify me at least 24 hours prior to your appointment if you need to cancel or change the time.
