Intake forms must be completed before the first session. You may complete them in advance and bring them with you (click to download or view and print), or complete them before the session in my office (I will supply copies). All new clients must complete the General  Information form, Memo of Understanding, Fee Schedule, and the HIPAA Notice. These forms are optional Release-Obtain Confidential Information - Adult, or Release-Obtain Confidential Information - Child, but if you know you will have to complete one (for probation, etc.) you may wish to do so in advance.

 

You will need Adobe Reader Installed on your computer (most have it installed by the manufacturer), just click the link and it should open.
